Staying safely in your home is our priority. Since 2020, PNW Home For Life has evaluated, modified, and adapted homes to promote functional independence, safety, and life satisfaction for older adults and community members with physical limitations.
A Six-Step Process, Start to Finish
From the first phone call to the final follow-up visit, every step is designed and overseen by a licensed occupational therapist.
Initial Assessment
Identifying client and family needs through an initial phone interview.
Occupational Therapy Evaluation
Evaluating accessibility needs and limitations through an in-home occupational therapy evaluation.
Planning
Creating a plan for remodel and/or installation of adaptive equipment, balancing safety, function, and aesthetics.
Implementation
Constructing proper home modifications with our network of vetted local contractors.
Client Education
Educating clients and families on how to properly use new modifications and equipment.
Follow-Up Visit
Confirming that solutions have met client needs and that training on new equipment is complete.

Home Modifications
Accessible remodels, wheelchair ramps, accessible kitchens and bathrooms, and additions designed for aging in place.
OT Home Evaluations
Client-centered evaluations covering fall prevention, safety, and accessibility, conducted by licensed occupational therapists.
Weekly Check-Ins
A 45-minute weekly visit for clients who want to stay independent, with safety checks and light household support.
Fall Prevention Training
In-home training and no-cost community trainings offered at regional hospitals and senior centers.

Coverage & Funding Options
Occupational Therapy Coverage
OT home safety evaluations, education, and hands-on training are typically covered by Medicare Part B (approximately 80% of cost); the remaining portion may be covered by supplemental insurance or out-of-pocket. Medicare Advantage coverage is evaluated case by case.
Home Modification Funding
Home modification work is generally paid out of pocket, and we help clients explore funding options, including:
- Community Development Block Grants
- Rural Housing Preservation Grants
- VA benefits
- Medicaid waivers
- Mortgage refinancing options
