Connie Colleen

Founder of PNW Home for Life, PLLC, is a Fidalgo Island native who loves exploring the area’s hidden gems with her better half and four children. Believing that nature boosts health and quality of life, their family aims to spend 1000 hours outdoors each year. Connie holds a B.S. in Psychology from BYU-Hawaii and an M.S. in Occupational Therapy from the University of Puget Sound. With seven years of experience, she specializes in home healthcare, home safety evaluations, and home modifications through her mobile outpatient clinic. Connie’s passion is to help families and seniors enjoy their favorite activities while balancing safety and independence.

PNW Home for Life is in network with Medicare Part B and will bill applicable services to patient’s Medicare. A great perk is there is no initial referral needed from your physician to be seen for an initial evaluation. We work alongside your physician if a plan of care is established after that initial evaluation.
